Topical Steroid Withdrawal: distinct condition or diagnostic grey zone?

A growing number of eczema patients are reporting having topical steroid withdrawal (TSW), a poorly understood condition that some say doesn’t truly exist, prompting others to scrutinize one of dermatology’s most widely used treatments. Topical corticosteroids (TCS) have been a mainstay therapy for eczema since the 1950s and remain recommended in clinical guidelines as a safe and effective way to control inflammation and flares. But in recent years, fuelled in part by social media platforms like TikTok, patients have increasingly described a constellation of symptoms after stopping prolonged steroid use. These include redness, burning, itching and skin sensitivity, sometimes severe enough to disrupt daily life and in some cases leading to a growing “steroid phobia.”The phenomenon, also referred to as “red skin syndrome” or “steroid addiction,” sits in a grey zone: increasingly discussed, sometimes recognized as distinct, poorly defined, under-researched and still contested.According to the Eczema Society of Canada (ESC), there are currently no universally accepted diagnostic criteria, and experts say its symptoms can closely resemble eczema itself, other skin conditions or allergic reactions, complicating diagnosis and treatmentESC cites a 2022 article published in the Journal of Dermatologic Treatment that says the phenomenon typically occurs after prolonged use of moderate to high-intensity topical corticosteroids on the face, and that this phenomenon is more common in women, and “many patients present due to improper use such as for cosmetic reasons.”Dr. Peter Lio, a Chicago-based dermatologist is the author of several scientific papers on TSW. He shared on the ESC site: “We don’t have accepted diagnostic criteria for TSW at this time, but we know that patients are reporting these symptoms. The good news is there are many other treatment options for eczema that aren’t from the corticosteroid class or family of medications.” 'Elephant skin'Beyond red, burning skin, some patients also report severe itching, peeling, swelling, oozing, and features such as “elephant skin,” referring to thickened, less elastic skin, and “red sleeve” (widespread redness along a limb), or additional symptoms on skin where corticosteroids were not applied. Some also report systemic effects, such as insomnia, fatigue and mood changes such as depression. In a recent clinical review in the Journal of American Academy of Dermatology, Canadian authors led by Pickering, Ont., dermatologist Dr. Kim Blakely, highlights how TSW remains both increasingly recognized but poorly defined, and subject to social media misinformation and far-fetched speculative theories, "including implausible underlying causes, such as 'a leaky gut' (with proponents often advertising expensive pseudoscientific consultations) and promotion of nonconventional treatment options." The paper also looks into the evolving understanding of TSW’s biology. Several mechanisms have been proposed but are unproven:Rebound vasodilation, possibly mediated by nitric oxide, leading to persistent redness and burningMitochondrial dysfunction, including altered NAD+ metabolismSkin barrier disruption and inflammatory cascadesEvidence on how long symptoms last is limited, but reports suggest they can persist for months to years, sometimes following a pattern of flares and periods of relative calm.Yet the review underscores a central problem: There is still no agreed-upon definition or diagnostic criteria, and symptoms frequently overlap with eczema, rosacea and contact dermatitis, making clinical distinction difficult. The debate leaves clinicians navigating a delicate balance: reassuring patients about the safety and effectiveness of topical steroids when used correctly, while acknowledging and investigating reports of symptoms seemingly from long-term use.For now, experts emphasize cautious use ensuring potency is matched with clinical need, limiting duration and ensuring patients understand both benefits and risks.
