For an upcoming feature series in 2026 in the Medical Post magazine, we are looking to profile the most effective and innovative team-based care models across Canada.

We know the landscape is shifting. From family health teams in Ontario to collaborative care clinics in the Maritimes and PCNs in the West, doctors are increasingly working shoulder-to-shoulder with nurse practitioners, physician assistants, and allied health professionals to manage complex patient needs.

We want to hear about teams that are working exceptionally well or doing something unique. Does your practice have a unique workflow with a PA? Have you integrated a social worker or dietitian in a way that has transformed your chronic disease management? Are you a rural team using allied health to bridge access gaps?

What we’re looking for:

  • Composition: Teams anchored by physicians but heavily integrated with NPs, PAs, pharmacists, social workers, etc.
  • Impact: Improvements in patient access, provider well-being, or specific health outcomes.
  • Uniqueness: Novel funding models, rural/remote solutions or specialized focus areas (e.g., mental health, addictions, elder care).
