Continuity of care died in the inbox, not the exam room

Ask a room of doctors what threatens the patient-doctor relationship and you’ll hear the usual suspects. Telemedicine. Walk-in clinics. Corporate medicine. Patients who doctor-shop. I used to nod along with all of it.Then a longtime patient of mine, someone I’d looked after for years, told me she’d started going to a walk-in for anything urgent. Not because she didn't trust me. Because, in her words, “your office is impossible to reach.” She’d called twice about a result. Left a voicemail. Got a callback while she was at work, missed it and the loop started over. From where I sat, nothing had gone wrong. Her result was normal, her file was fine and the medicine was solid. From where she sat, her doctor had gone quiet.That conversation changed what I believe about continuity of care. I now think continuity doesn’t break in the exam room. It breaks in the inbox, the voicemail box, the recall list nobody ran this month and the rebooking that fell through the cracks. And if that’s where it breaks, then protecting it is an operational job. My operational job.What continuity actually looks like from the patient’s sideWe talk about continuity as a relationship: same doctor, over time, who knows your story. That’s the part we control in the room, and most of us do it well.But between visits, the patient doesn’t experience "the relationship." They experience our systems. The phone line that rings out at lunch. The result sits in a queue. The recall for a follow-up that never went out because it lived in a different program than the booking calendar. The message they sent through one portal while the clinic was watching another.In my own practice, I counted the places a patient interaction could live: the phone system, the fax, the EMR inbox, a results queue, a recall spreadsheet and the front desk’s memory. Six systems. None of them talked to each other. A patient with a simple question could touch three of them and still get no answer. To her, that reads as one thing: my doctor’s office doesn’t have it together. The clinical relationship I’d spent years building was being judged, fairly, by the weakest administrative link.Why I stopped calling this a scheduling problemFor a long time, I filed all of this under scheduling. Not enough slots, not enough staff, too many calls. So the fixes I reached for were scheduling fixes: another admin shift, a longer phone rota and double-booking buffers.None of it moved the needle much because the problem wasn’t volume. It was fragmentation. A message that arrives in a channel nobody owns will be missed at any staffing level. A recall list that lives outside the booking system will leak patients, no matter how many hands you hire. We weren’t understaffed so much as scattered.Once I saw it that way, the question changed. Instead of “how do we handle more calls,” it became “how many places can a patient’s request land, and who owns each one?” The honest answer embarrassed me. Some channels had no owner at all. We’d built a clinic where a patient could do everything right, call, message, wait patiently and still fall into a gap between two systems that each assumed the other had it.The part doctors don’t like to hearHere’s my argument, plainly. Continuity of care is now an operations problem, and operations belong to us. Not to the patient, who can’t fix our phone tree. Not to the system at large, which will not redesign our workflows for us. To us, the physicians who own or run these clinics.I understand the resistance, because I felt it. We trained for medicine, not logistics. It’s tempting to treat the administrative layer as beneath the clinical work, something to delegate and forget. But if a patient’s experience of my care is shaped by whether her result reached her, whether her follow-up got booked, whether her question got answered within a week, then that layer is not beneath the clinical work. It carries the clinical work.The changes that helped us weren’t dramatic. Fewer places for a land request. A named owner for every channel. Recalls tied to the same calendar that books the appointment, so a follow-up is created the moment it’s ordered rather than remembered later. A way for patients to get an answer without winning the phone lottery. Boring fixes. But my no-answer complaints dropped, and the patient who’d drifted to the walk-in came back.(The patient described above is a composite of the kinds of patients I see, with details changed. Not one specific individual.)What I’d say to a colleagueIf your patients are drifting, look at your gaps before you blame their loyalty. Map every place a patient contact can land in your clinic and ask who owns it. If the answer is “sort of everyone,” it’s no one.We didn’t lose continuity to telemedicine or to walk-ins. Those are symptoms. Patients go around us when going through us stops working. The relationship in the exam room is still strong.
