Among them, a significant subset—estimated at between 150,000 and 465,000 Canadians—grapples with severe asthma, a debilitating form of the disease that often remains uncontrolled even with conventional high-dose treatments such as inhaled corticosteroids (ICS), LABA, and LAMA. The impact on quality of life is profound. Patients with severe asthma often face a relentless cycle of symptoms, exacerbations, and hospitalizations, leading to fear, anxiety, and frustration. Simple daily activities become monumental challenges, affecting school attendance for children and productivity for adults.The economic burden on Canada's healthcare system is equally staggering. Asthma alone costs the Canadian economy an estimated $2.1 billion annually, a figure projected to rise to $4.2 billion by 2030 if current trends continue. In 2021, asthma attacks resulted in more than 80,000 emergency room visits across the country. These statistics underscore a critical reality: despite existing treatments, many Canadians are not achieving optimal disease control. A 2016 survey indicated that 90% of Canadians with asthma reported symptoms showing their condition was not well-controlled, even if they perceived it to be. More recently, Asthma Canada's 2025-2030 Strategic Plan highlighted that shockingly, half of people living with asthma still live with uncontrolled asthma, leading to preventable suffering and substantial healthcare resource utilization. What these numbers reflect is not a lack of therapeutic options, but a persistent gap between disease biology and treatment approach, particularly for patients with Type 2-driven inflammation who remain exposed to recurring exacerbations and systemic steroids.The Avoidable Cost of Corticosteroid Dependence For decades, corticosteroids, both inhaled and oral, have been the cornerstone of anti-inflammatory therapy in respiratory diseases. They are invaluable for managing acute exacerbations and maintaining control. However, the long-term and repeated use of oral corticosteroids (OCS) comes with a significant cost to patient health. The cumulative negative effects are well-documented and can profoundly diminish a patient's quality of life.Chronic OCS exposure is associated with a wide array of adverse effects, including weight gain, fluid retention, increased blood sugar levels, high blood pressure, and bone thinning (osteoporosis), which elevates the risk of fractures. Patients may also experience increased susceptibility to infections, mood swings, cataracts, glaucoma, insomnia, irritation, confusion, and, in some cases, even depression or psychosis. These side effects can manifest even at low doses and significantly impact a patient's overall well-being and adherence to treatment.A recent Canadian real-world evidence study—the ALERT (chAracterizing uncontroLled sevERe asThma in Canada) study—starkly highlighted this challenge. It revealed that 40% of patients with uncontrolled severe asthma had received four or more OCS courses, and a concerning 72% of these patients were not on available biologic therapy. This data points to a clear pattern of OCS overuse and a significant gap in the adoption of more targeted, steroid-sparing treatments. Frequent corticosteroid exposure is no longer an acceptable trade-off in patients with biologically driven disease. Today, it represents a missed opportunity to intervene earlier with targeted, steroid-sparing therapies that modify risk over the long term.A New Horizon: Targeted Innovation and the IL-5 PathwayThe evolution of respiratory medicine has brought forth a new era of targeted therapies, particularly biologics, which represent a paradigm shift in managing severe respiratory conditions like severe eosinophilic asthma. In patients with this phenotype, elevated IL-5 levels drive eosinophilic inflammation in the airways, leading to recurrent exacerbations and persistent symptoms.IL-5 inhibitors are a class of biologics designed to interrupt this specific inflammatory cascade. By either binding directly to IL-5 (e.g., mepolizumab, reslizumab) or blocking its receptor (e.g., benralizumab), these therapies prevent IL-5 from exerting its effects on eosinophils. By targeting IL-5 upstream in eosinophil biology, these treatments offer substantial clinical benefits: they have been shown to significantly reduce asthma exacerbations, improve lung function, and, importantly, reduce reliance on oral corticosteroids, thereby mitigating their associated side effects. For patients who have long struggled with refractory disease, this approach delivers predictable, sustained control of inflammation and a chance at a dramatically improved quality of life. Bridging the Gap: Ensuring Innovation Reaches PatientsThe availability of these life-changing innovations is a testament to scientific progress. However, the journey from discovery to patient benefit is not always straightforward. As the ALERT study indicated, a significant proportion of eligible patients in Canada are not yet receiving these advanced therapies.
