B.C. physician master agreement delivers major boost to rural medicine

British Columbia's new physician master agreement (PMA) is being described as a significant step forward for rural medicine, with Doctors of BC highlighting new investments aimed at strengthening recruitment, retention and physician support outside the province's urban centres.In an interview with the Medical Post, Doctors of BC president Dr. Adam Thompson said he viewed the agreement through the lens of his own rural practice.“I work in a rural community, just outside Courtenay in the Comox Valley, and I think seeing increases to support rural health care is crucial,” he said.Dr. Thompson also highlighted increased funding for rural health, higher after-hours and medical on-call premiums and greater support for rural practice business costs, describing the agreement as “a strong step forward for doctors, but also for the healthcare system and patients.”Dr. Birinder Narang, president of BC Family Doctors, echoed that assessment. In an interview with the Medical Post, he said the agreement included.support for physicians new to rural communities increased funding for the Joint Standing Committee (JSC) on Rural Issuesenhancements to rural locum programsmore stable funding for virtual specialist support programs such as Child Health Advice in Real-Time Electronically (CHARLiE) and Maternity and Babies Advice Line (MaBAL) improvements to alternate payment contractsa commitment to improve the Longitudinal Family Physician (LFP) payment model for rural practice.“There had been uncertainty with the MaBAL and CHARLiE programs specifically, but the PMA funding covers the cost of converting the current program contracts to a 24/7 scheduled-shift model, so there’s more predictability and sustainability in accessing those programs,” Dr. Narang said.What rural physicians can expectUntil last week, the full 314-page agreement had been available only to Doctors of BC members. It has now been released publicly.The agreement provides $25.75 million a year in new and ongoing rural funding beginning April 1, 2026, followed by a further $12 million annually beginning April 1, 2027 and a further $15 million annually beginning April 1, 2028.It also includes $4.75 million in one-time funding for the JSC to support priorities including rural practice business costs, rural locum enhancements, physician recruitment, improvements to the rural retention program with its medical isolation-point system and other rural initiatives.One of the most tangible benefits is for physicians working under general practice full scope service contracts in designated rural areas A, B and C.Beginning April 1, 2026, the agreement raises the maximum value of those contracts by $85,490 annually, with proportional increases to the minimum contract values and salary agreement ranges. While not every rural physician will receive the full amount, it represents one of the clearest examples in the agreement of a direct increase in rural physician compensation.(The agreement does not specify how much individual physicians will receive under many of the new initiatives, leaving much of the implementation to the JSC.)Rural docs and the LFPBC Family Doctors previously ran an advocacy campaign that included the need for changes to the LFP model, arguing that features such as its minimum patient-panel requirements disadvantaged physicians serving small or seasonal communities.The PMA acknowledges the need to review the model but leaves that work to the JSC.Will the PMA serve as a draw to rural medicine?While many implementation details remain to be worked out, the new B.C. agreement formally commits actual funding and policy attention to rural medicine—an area that has struggled with recruitment and retention for years, not just in B.C. Both Dr. Thompson and Dr. Narang said they hoped the package would make British Columbia increasingly competitive in attracting rural physicians.“We’ve come a long way in the last few years, and I think this PMA will continue to help bolster that,” Dr. Narang said.
